CLIENT CONSENT & LIABILITY WAIVER

Please read carefully before receiving any services at Adonis Bodyworks.

I, the undersigned, hereby acknowledge and agree to the following:

1. I understand that the services provided by Adonis Bodyworks include massage therapy, facial treatments, trim shaving services, and optional add-ons such as cupping therapy, hot stone therapy, essential oils, and CBD oil.

2. I confirm that I have disclosed all relevant medical conditions, allergies, skin sensitivities, injuries, or health concerns prior to receiving any service. I understand that failure to do so may increase the risk of adverse reactions.

3. I understand that trim shaving and services involving private or sensitive areas may involve minor risks such as skin irritation, redness, ingrown hairs, or discomfort. I voluntarily consent to receive these services.

4. I acknowledge that cupping therapy, hot stone therapy, and deep tissue massage may result in temporary marks, soreness, or bruising.

5. I agree that Adonis Bodyworks, its owners, employees, and therapists shall not be held liable for any injury, allergic reaction, or complication resulting from services provided, except in cases of gross negligence.

6. I understand that spa services are not a substitute for medical treatment and that no diagnosis or medical advice is being provided.

7. I voluntarily assume full responsibility for my participation in all treatments and hereby release and discharge Adonis Bodyworks from any and all claims, demands, or causes of action arising from my participation in these services.

8. I confirm that I am at least 18 years of age and legally able to consent to treatment.
